Mileage Reimbursement Form

From Bob Young
For those applying for In-District Mileage Reimbursement

If you are using your own vehicle for in-district travel required by your position responsibilities, you will need to use the form provided by the district to report that mileage. This is the required reporting form beginning in October 2015. This form can be found here.

Instructions are provided on the cover of the form, which includes the use of the most recent version of Acrobat Reader. Instructions on how to download this software are on the cover page. This form is self-calculating and allows for multiple pages of reporting.

This should be submitted monthly and be reported by the calendar month. Again, forms must be submitted within 60 calendar days of the oldest date on the report to be considered for reimbursement. Addresses of destinations not on the register of district facilities must be clearly identified.

Please make sure that any employees that are under your direction are aware of this requirement. Thank you for your cooperation in this matter.
